Once approved, you will receive your certificate and a waiver via email. You must sign the waiver in the presence of a notary (you may use an online notary service) and send the notarized waiver back to Leafwell via email. Proof of Identity
Acceptable forms of ID include a Georgia driver’s license or state ID.
The following are also accepted with two proofs of residency: Out-of-State Driver’s License or State ID, US Passport, Military ID, US Birth Certificate, Official Consular Report of Birth Abroad (FS-240, FS545, or DS-1350), Certificate of Naturalization (N-550, N-570), Certificate of Citizenship (N-560, N-561).
