Caregiver
Caregivers must be 21, complete the caregiver application, and register with the Massachusetts Cannabis Control Commission’s Medical Use of Marijuana Program. You’ll also need the PIN from the medical patient.
Can Minors Qualify?
Minors can have a parent or guardian become their caregiver and apply for a medical cannabis card on their behalf.
The minor must have certifications from two healthcare providers, one being a pediatrician or pediatric specialist.

How does the online evaluation process work for getting a medical marijuana recommendation letter/medical certificate in Massachusetts?
- Register with Leafwell and speak to a Massachusetts-licensed healthcare provider via our HIPAA-compliant telehealth platform, which involves creating a Leafwell account using a valid email address. You'll be asked to complete a brief questionnaire of personal and health information and to either take or upload a photograph of identification (we're required by law to verify your identity). You will need a smartphone, computer, or tablet with a reliable internet connection and a working camera to be seen by a Leafwell provider.
- Schedule an appointment or enter the virtual waiting room. You will receive a text message on your cell phone when the provider is ready to see you. You'll be connected to a Leafwell healthcare provider for a video chat (i.e., a virtual doctor's visit). During the visit, your provider will assess your medical condition(s) to determine if you qualify for a Massachusetts medical marijuana certification.
- Once you're approved, we'll send you a copy of your certification via email, which you'll need to register with the Massachusetts Medical Use of Marijuana Program through the state's Cannabis Control Commission. Use the PIN on your certificate to complete and submit your state registration. When your state registration is approved, you'll receive a printable Temporary Program ID Card (valid for 42 days) to purchase medical cannabis, while your plastic Program ID Card will arrive in the mail in one to two weeks.
How old do I have to be to apply for a medical marijuana card in Massachusetts?
Can I be/have a caregiver for a medical marijuana patient in Massachusetts?
- 21 years of age or older.
- Patients can designate up to two caregivers. Caregivers can serve up to five patients.
- An employee of a hospice, nursing, or medical facility who's providing care to a qualifying patient, may serve as a designated caregiver; including, to patients younger than 18 as a second caregiver.
- Must carry medical ID card at all times.
- Notify the state within 5 days if the card is lost, stolen, or destroyed: 617-660-5370
- After obtaining a PIN from your Patient, log on to the Online System.
- Click the "Register as the Caregiver of an Adult Patient" button on the home page.
- If the patient is a minor, click the "Register as the Parent or Legal Guardian of a Pediatric Patient (Under 18)" button.
- Enter your identifying information.
- On the next page, you will see instructions for registering. After reading the instruction page, click "Proceed" to continue the application.
- On the following page, you will enter your personal information in the provided field. For renewals, your current registration information will automatically appear in the application fields.
- Continuing on the same page, you will read the attestation and select the checkbox to agree to the attestation.
- Next, you will click the "Proceed" button to continue.
- Enter your ID details.
- If you selected a State License or State ID for the form of identification and the system is unable to retrieve your photograph from the Massachusetts Registry of Motor Vehicles (RMV), you will next go to the Identification Verification page. Review your information and correct any incorrect items. Click the "Proceed" button to continue.
- If the identification being used is not from Massachusetts, click the "Proceed" button.
- On the next page, enter your identification document expiration date, select the file containing your scanned identification document, and upload it by clicking the "Upload" button. You will then see the document listed in a table above the form. To continue, click the "Proceed" button.
- On the next page, you will select the file containing your identification photo and upload it by clicking the "Upload" button. If the system was able to obtain your photograph from the Massachusetts RMV, you will see the photo displayed.
- If the system is able to obtain the photo after you upload a photograph, you will have the option to use this photograph. You then may click the "Proceed" to continue.
- On the next page, review your application information and then click "Submit" to submit your application. If any information needs to be updated, click on the piece of information to return to the page where it may be updated.
- Once you have registered, your Patient must approve your caregiver status by logging into the Online System.
- Once that is completed, the Program will process your registration.
How do I apply for the state program in Massachusetts?
- Your PIN (provided by Leafwell)
- A valid form of identification
- Massachusetts driver's license
- Massachusetts ID card (with a photograph of yourself)
- U.S. passport and proof of MA residency
- U.S. military ID and proof of MA residency
- A photograph of yourself if you are not using a Massachusetts-issued driving license or ID.
- Proof of Massachusetts residency if a U.S. passport or U.S. Military ID is to be used as the form of identification
- Visit the Medical Use of Marijuana Program Online System of the Cannabis Control Commission at https://patient.massciportal.com and create an account. Click on the "Create Account" link and receive a registration email to get started.
- Log back into your state account and click the "Register as an Adult Patient" button.
- Enter all four identification fields and click the "Proceed" button. The PIN Number is listed on your Leafwell certification.
- Read through the registration instructions and then begin filling in your information as prompted.
- Upload a photo of your valid ID. If you use a U.S. passport or U.S. military ID, you'll need to also upload a passport-style photo of yourself for your medical card and then your proof of Massachusetts residency.
- Review and submit your application. Once your registration is approved, you can print a temporary, paper Program ID Card that is valid for 42 days. Your plastic Program ID Card should arrive within 1-2 weeks
